Zachary Turner 8778fea0bb Convert all use of pthreads in tests to c++11 threads. 
This eliminates portability issues among platforms that don't have
a pthreads implementation.

Daniel Malea 7987088a8a Add tests for 'thread step out' for multithreaded programs 
- test with python API
- test with command interpreter
- test stepping a single (selected) thread
- test stepping all threads in the program
