Add the `memory_scope_all_devices` enum value, which is restricted to
OpenCL 3.0 or newer and the `__opencl_c_atomic_scope_all_devices`
feature. Also guard `memory_scope_all_svm_devices` accordingly, which
is already available in OpenCL 2.0.
The `__opencl_c_atomic_scope_all_devices` feature is header-only, so
set its define to 1 in `opencl-c-base.h`. This is done
unconditionally at the moment, as the mechanism for disabling
header-only options hasn't been decided yet.
This patch only adds a negative test for now. Ideally adding a CL3.0
run line to atomic-ops.cl should suffice as a positive test, but we
cannot do that yet until (at least) generic address spaces and program
scope variables are supported in OpenCL 3.0 mode.

Ensure that the cl_khr_3d_image_writes pragma is enabled by making
cl_khr_3d_image_writes an optional core feature in CL 3.0 in addition
to being an available extension in 1.0 onwards and a core feature in
CL 2.0.

Extended subgroups are library style extensions and therefore
they require no changes in the frontend. This commit:

Rename lang mode flag to -cl-std=clc++/-cl-std=CLC++
or -std=clc++/-std=CLC++.

Prior to OpenCL 2.0, image3d_t can only be used with the write_only
access qualifier when the cl_khr_3d_image_writes extension is enabled,
see e.g. OpenCL 1.1 s6.8b.
Require the extension for write_only image3d_t types and guard uses of
write_only image3d_t in the OpenCL header.

This header file is based on the Khronos implementation (https://github.com/KhronosGroup/SPIR/blob/spirv-1.0/lib/Headers/opencl.h) with heavy refactoring.
