The C headers are deprecated so as requested in D102845, this is replacing them
all with their (not deprecated) C++ equivalent.

This patch introduces a LLDB_SCOPED_TIMER macro to hide the needlessly
repetitive creation of scoped timers in LLDB. It's similar to the
LLDB_LOG(F) macro.

This is a step towards making the initialize and terminate calls be
generated by CMake, which in turn is towards making it possible to
disable plugins at configuration time.
