If a test does not contain an " simd" but -fopenmp-simd RUN lines we can
just check that we do not create __kmpc|__tgt calls.

If the loop counter is not declared in the context of the loop and it is
private, such loop counters should not be captured in the outlined
regions.

OpenMP creates a variable array type with a a null size-expr. The Debug
generation failed to due to this. This patch corrects the openmp
implementation, updates the tests, and adds a new one for this
condition.
