This reverts commit 1e1f752027.
It breaks ignore_noninstrumented_modules=1.
Somehow we did not have any portable tests for this mode before
(only Darwin tests). Add a portable test as well.
Moreover, I think I was too fast uninlining all LibIgnore checks.
For Java, Darwin and OpenMP LibIgnore is always enabled,
so it makes sense to leave it as it was before.

LibIgnore is checked in every interceptor.
Currently it has all logic in the single function
in the header, which makes it uninlinable.
Split it into fast path (no libraries ignored)
and slow path (have ignored libraries).
It makes the fast path inlinable (single load).

size() is inconsistent with length().
In most size() use cases we can replace InternalScopedString with
InternalMmapVector.
Remove non-constant data() to avoid direct manipulations of internal
buffer. append() should be enought to modify InternalScopedString.
- Fixing VS compiler and other cases settings this time.
