As part of the unification of the debug format and the MIR format, print
MBB references as '%bb.5'.
The MIR printer prints the IR name of a MBB only for block definitions.

There is no situation where this rarely-used argument cannot be
substituted with a DIExpression and removing it allows us to simplify
the DWARF backend. Note that this patch does not yet remove any of
the newly dead code.

Avoid duplicating DictScope with hand-written names everywhere. Print
the S_-prefixed symbol kind for every record. This should make it easier
to search for certain kinds of records when debugging PDB linking.

For small, discontiguous local variable regions, CodeView can use a
single defrange record with a gap, rather than having two defrange
records. I expect that this optimization will only have a minor impact
on debug info size.
